Causes of Female Infertility

Female infertility has many causes - structural, hormonal, or lifestyle-related - and identifying the root cause is key to effective treatment.

At Dr. Rama Sofat Hospital, we evaluate each patient comprehensively to find the right pathway forward, whatever the cause.

The Causes

01 — OVULATION

Ovulation problems

The egg does not release properly each month, so there is nothing to fertilise.

  • PCOS
  • Thyroid issues
  • High prolactin
02 — TUBES

Blocked tubes

The fallopian tubes are the egg's path. If blocked by infection or scarring, fertilisation cannot happen.

  • PID
  • TB
  • Old surgery
03 — UTERUS AND CERVIX

Uterus or cervix issues

Structural problems like fibroids, polyps, or scar tissue can stop the embryo from implanting.

  • Fibroids
  • Polyps
  • Scar tissue
04 — ENDOMETRIOSIS

Endometriosis

Uterine-like tissue grows outside the uterus, causing inflammation and distorting the reproductive organs. Often missed.

  • Inflammation
  • Cysts
  • Adhesions
05 — HORMONES

Hormonal imbalance

Hormones like FSH, LH, estrogen and progesterone must be in balance. Any disruption can prevent ovulation or implantation.

  • FSH and LH
  • Estrogen
  • Progesterone
06 — AGE

Age

Egg quality and quantity naturally decline over time.

20–25% chance per cycle in 20s and 30s
~5% chance per cycle in early 40s
07 — LIFESTYLE

Lifestyle factors

Daily habits affect hormones and egg quality, but many are reversible with the right support.

  • Smoking
  • Alcohol
  • High stress
  • Weight
08 — UNEXPLAINED

Unexplained infertility

In 10–15% of couples, all tests come back normal. Treatment is guided by age and history, often starting with IUI then IVF.

  • IUI first
  • Then IVF or ICSI
09 — RECURRENT MISCARRIAGE

Recurrent miscarriage

When two or more pregnancies are lost before 24 weeks. Conception happens, but the pregnancy cannot continue. Treatable with targeted care.

  • Genetics
  • Autoimmune
  • Hormonal
